IV Therapy Appointments

A $75 deposit is required to confirm every IV appointment. The deposit is applied to your total.

Cancellations or changes made at least 12 hours before your appointment receive a full refund of the deposit, or it can be moved to a new date.

Cancellations made less than 12 hours before the appointment, and no shows, forfeit the deposit.

Arriving 20 to 60 minutes late includes a $50 late fee. Delays of more than 1 hour count as a missed appointment.

Travel is free within 15 miles of our Edgewater base. A $30 transportation fee applies beyond 15 miles. Parking, valet or building access fees are the client's responsibility.

Consultations

All consultation fees are non refundable once the consultation has taken place. You may reschedule up to 24 hours before your appointment at no cost. Missed consultations or cancellations with less than 24 hours notice are non refundable.

Peptide Protocols and Prescribed Products

All prescribed products and protocols are final sale and non refundable. For safety reasons, prescribed products cannot be returned or exchanged once dispensed.

Shipping

Prescribed products are shipped directly by our licensed partner pharmacy with tracking. Please make sure your shipping address is correct; we are not responsible for orders sent to an incorrect address provided by the patient.

Once tracking shows the package as delivered, Vita Wellness by C.J. is not responsible for lost, stolen or damaged packages.

If your package is delayed, missing or arrives damaged, contact us within 48 hours of the expected delivery date at info@vitawellnessbycj.com and we will work with the pharmacy and the carrier to resolve it.
